New Patient Information
Your First Visit
Your initial appointment will consist of an interview with one of our dental hygienists to discuss your concerns followed by an examination, cleaning of your teeth (unless a finding of gum disease is noted), and an explanation of our observations. Dr. Flex or Dr. Hodge will then discuss their recommendations for any necessary treatment as well as your options and costs. A person with an extensive medical history or complex dental issue may require further evaluation and a second appointment so that we can thoroughly review their condition.
Please assist us by providing the following information at the time of your initial appointment:
- X-rays if you have them and they are less than one year old
- The name and address of your previous dentist so we can obtain any pertinent information or x-rays
- A list of medications you are presently taking
- If you have dental insurance, please bring your insurance card with you. This will save time and allow us to help you process any claims.
If your former dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office if possible. If additional films are necessary, they can be taken at our facility.
IMPORTANT: All patients under 18 must be accompanied by a parent or guardian during their initial office visit.
Please alert the office if you have a medical condition that may be of concern i.e. a history of mitral valve prolapse, diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc. or if you are on any medications such as heart medications, aspirin, anticoagulant therapy, etc.
For your convenience, we’ve made our patient forms available online so that you can fill them out before your appointment. This way, you can show up to our office and you’ll be able to see your doctor almost immediately.

We will gladly accept most dental plans, including those from MetLife, Delta Dental, and Cigna. We can provide you with a free benefit analysis, which will assist you in understanding your individual insurance coverage. However, we do require payment of the estimated co-payment portion of your bill at the time of service.
After your dental plan processes your claim, you will be responsible for any remaining balance. Your policy is a contract between you and the insurance company, and we are not a party to that contract. If your dental plan has not paid your account in full within 45 days, the balance must be paid once you receive your billing statement.
Please be aware that some, and perhaps all of the services provided may be non-covered services and may not be considered reasonable and conventional under your dental plan. Our practice is committed to providing excellent patient care, and our charges are usual and customary for our area.
You are responsible for payment, regardless of a dental plan’s arbitrary determination of usual and customary rates. Please be advised that if your treatment is not covered under your specific plan, full payment will be due at the time of service.
